Adejoke Adedeji APRN PMHNP-BC
Board Certified Psychiatric Mental Health Nurse Practitioner

Adejoke has an extensive education in health care. She began by graduating with a bachelor’s degree in kinesiology and exercise science from Concordia University in St. Paul, Minnesota. She went on to complete her Master’s of Public Health at Florida International University in Miami. She then earned her bachelor’s degree in nursing at Valdosta State University in Valdosta, Georgia, and her Master’s of Nursing, focusing on psychiatry and mental health, at the University of Tennessee in Knoxville. 

 

Enitan Ekwotafia, APRN PMHNP-BC
Board Certified Psychiatric Mental Health Nurse Practitioner

Enitan O. Ekwotafia is a board-certified psychiatric mental health nurse practitioner with experience treating patients with psychiatric conditions from adolescence to geriatrics. She has over 18 years of varied nursing experience. Her educational background includes both Masters and Doctoral degrees from the University of Florida, with a postgraduate specialization in psychiatric mental health nursing from Florida State University. She is an active member of the American Academy of Nurse Practitioners and the American Psychiatric Nurses Association.

Psychiatric Mental Health Nurse Practitioner

Barb has been working in the field of Psychiatry for approximately 10 years now. She loves working with people of all ages and is excited to bring her passion to work for Nexus. Barb graduated from Vanderbilt University in Nashville TN and has resided in FL since 2007. When she is not helping to change patients’ lives, she loves spending time either on the beach or standup paddle boarding. Let Barb help you on your path to inner peace!

Blake Johnson MA, LADC

Blake Johnson is a military veteran who is worked over 10 years with individuals of various ethnic and social-economic backgrounds, including children, adolescents, and adults.  Blake enlisted in 2010-2011 as a religious affairs specialist and mental health liaison and served over 6 years in the Army.  Blake has worked in many other positions, both during and after his service, some of which include mental health practitioner, chemical dependency counselor (LADC), behavior analyst/technician, direct support professional, and most recently an independent living skills specialist.  Blake graduated from Winona State University in 2014 and received his undergraduate degree in Sociology: Criminal Justice Corrections (SCJC) with a minor in Child Advocacy Studies (CAST) and was certified as a child interviewer.  After interning for the Winona County Drug Court, he applied for, and was accepted to, Saint Mary’s University of Minnesota and graduated in 2017 with a dual degree in Counseling and Psychological Services M.A, and received an Addiction Studies Graduate Certificate simultaneously.  Blake would go on and become a Licensed Alcohol and Drug Counselor (LADC) receiving his certification from the Board of Behavioral Health and Therapy (BBHT) and worked for an intensive outpatient treatment center in Burnsville thereafter.  Blake practices person-centered therapy and has an unconditional positive regard for those he works with.  His goal is to be used as a resource and promises to never give up on providing people with the opportunity for change and growth. 

 


 

Sarah Plank

Sarah Plank is a healthcare professional who has worked in the medical field for the last 8 years in primary care settings at M Health Fairview and Mayo Clinic Health Systems. Providing patients with the best possible care is a passion of Sarah’s.

Sarah grew up in Northfield where she continues to live. She enjoys spending time with her children, significant other, and two dogs, Leonard and Sidney.
